St. Maarten Medical Center Placed Under Higher Supervision by Inspectorate as of September 8.

smmc23082012The Inspectorate for Public Health, Social Development and Labor (VSA) conducted a governance audit of the Sint Maarten Medical Center (SMMC) and thematic site inspections at the end of 2011 to the first half of 2012.
Quality of care, internal supervision (governance), the surgical operations, the emergency department and the handling of accidents and incidents were audited through inspection visits and conversations with staff members, the board of directors and the supervisory council. For the audits on governance and surgical operations support was provided by the Inspectorate for Healthcare from Holland (IGZ).
The Inspectorate is aware of the fact that Sint Maarten is a small island with only one hospital, the SMMC. Demanding of this hospital that it complies with all requirements as stipulated for hospitals in the western world would be unrealistic and not feasible, but one must expect the SMMC to comply with basic quality –and patient safety standards and health care ordinances and regulations.
The audit and inspections leave much to desire. A common and important finding is the lack of proper communication in different areas and at different levels resulting in severe delay in the development of a basic quality- and safety system and a high risk for the patient. This situation is worsened by the fact that the Board of Directors is frequently (50% of the time) off island and replaced by a staff member without mandated authority and an incomplete Supervisory Council that cannot exercise its supervisory role adequately.
Also included in this report are some investigative results of incidents that occurred in 2010, 2011 and 2012, because they provide insight in the way the SMMC is organized. On one hand about the communication amongst professionals, and amongst professionals and the board of directors, and on the other hand how transparent the management is towards external supervision.
When publishing a report the Inspectorate takes into account the norms as prescribed by legislation such as the "Landsverordening Openbaarheid van Bestuur". This means that wherever possible personal information will not be mentioned in this report e.g. patient name and staff members of the SMMC will be mentioned as much as possible by their function.
Due to the Board of Directors not being very cooperative, it wasn't until July 12th 2012 the Inspectorate could speak with the director. The provided information has been processed in this report.
The Inspectorate uses the following ordinances and regulations for this audit and measures to be taken;

The findings of this audit have been compiled in a draft report which was delivered on August 24th to the board of directors of the SMMC for comment prior to publication. The deadline to deliver any comment to the Inspectorate was September 7th. On September 7th the board indicated in a letter to the Inspectorate that they couldn't comply with the deadline due to several internal delays and requested an extension till September 17th. The inspectorate denied this request and indicated that possible comments will be dealt with when discussing the action plan to be drafted.
Based on the findings and measures that need to be taken with regard to quality of care and patient safety, some on short term and others within a year, the Inspectorate has put the SMMC under higher supervision as of September 8th 2012.
This means that the board of directors must present a feasible action plan within two weeks to address many of the critical issues found and that the Inspectorate will be conducting announced and unannounced inspections at least three times a month following up on the action plan. Failure to comply may result in (temporary) closure of specific functional units.

    People, let us not look for quick fix. It will take some time to get it right. Approach it from this angle...long term and short term plan. For those urgent matters just present a "temporary urgency plan".

    This report does not only reflects badly on the 'independent' foundation but also reflects badly on government as a whole. Who are the victims? The victims/patients.

    Let us just start working together to ensure the people that they need not worry but can trust in the Lord that government and SMMC will some how get it right.

    The lord will help those who help themselves. there is no point in sitting in disbelief after so many reports audits, complaints, incidents, absence of director two weeks a month for the last two years,Yet we want to blame government, SMMC has been a private foundation for the last few years. And because of this setup, government no longer knew what was going on internally, because supervisory board and that scamp of a director made sure keep the info inside. Best thing government can do now is to start looking for people of integrity to serve on a new board and send Scot on a one way ticket to Holland.
    For years people on sxm been complaining of the lack of proper and effective health inspection. Finally we got it, and we still think it is politics.Anyone reading the stories coming from the smmc would be completely puzzled why the director still not gone. Something down there must be sweet why he ain't leavin

    Let's wait for SMMC's response. Please don't jump to conclusions. Your innocent untill proven guilty.
    May be Scott was going away to do some studies to enhance the SMMC?
    How many of similar deaths other hospitals has in comparison with SMMC?
    Are the number of deaths within range as compared with international bench markets?
    What was the role of the Health Department? How come they didn't have proper mechanisms in place or was the Health director only busy screwing her subordinates and collegeas?
    May be the Health Department needs an audit themselves?

    Direct government involvement has never been nor ever will be the magic solution for situations whereby either an institution or its board or its management faces challenges. Government itself seems unable to properly manage its organization, albeit there are structures in place for such. The approach to be followed should be one of prudence and placing the health interests of the public above all other considerations. This is possible to achieve, once government (read minister of public health) has a public health policy and once there is a level playing field for all actors involved.

    A word of advice to the government of SXM. Bring back doctor Mercelina and doctor Petit to run the St.Maarten Medical Center. Lets use our own, who been crying out for years about the bad management of SMMC.

    So Mr Observed: Are you saying that Scot does not has the appropiate diploma's , qualifications to be there as medical direcftor? Or do I now undertsand that the frequent trips to Holland is to get a degree in running a hospital? And is this not the man that collecting multiple salaries in the hospital? WOW!

    This is beyond real. Can't believe this still happening in these these with so much checks and balances. Or does Scot have a few compientjes working with him? Scot ;YOU are THE BEST

    Government will learn that before critisizing others they need to look at themselves. When you live in a gla** house as the calyso song says, don't throw stones. @hopewish, your comment is the best I have heard since this saga start. De Weever very fast with pointing fingers. But government need to start looking at their own hot mess in health care and their responsibility for health care. This grandstanding is just a minister trying to get sympathy for his misses. Government want to show what others do wrong but be very carefull their own flaws become public.

    O Please , give me a damn break. Closing certain units in SMMC ( ER, ICU, OR) for only an hour will result in you all falling down dead as flies!! So shut the f**k up, yall don't know what you talking about. Close those units for only 1 hour and yall will be talking a different language and begging hospital to open the units again.

    When you screw up at work, your job's on the line. When doctors of the SMMC screw up, somebody else's life is on the line. Talk about serious pressure. Nevertheless, on Little S’Martin the doctors that kill people just hop on a KLM/Air FRANCE flight and go back to Holland staying Scott FREE of the justice system

    Clarence Richardson thinks that by jumping ship early he got away or getting away. It is said
    that he collected well in the range of US$ 250.000 in Board and Consultancy fees during the year. Do what you have to do Mr Minister. Check them out and clean up the mess.

    10% doctors earn over accepted norm (FD)

    Some 650 medical specialists employed by teaching hospitals earn more than the €181,000 or US$ 231,503 per year limit set down for public sector salaries, reports Thursday’s Financieele Dagblad.
    According to a survey by the paper, some 10% of specialists earn more than the so-called Balkenendenorm which is based on Dutch ministers’ salaries.
    Since 2006, teaching hospitals have been obliged to make top salaries public because they are financed by public funds.

    Linking the pay of top civil servants to that of prime minister Jan Peter Balkenende is 'unwise', Roel Bekker, secretary-general at the home affairs ministry, said in a speech at Leiden University on Monday evening.
    The government wants top civil servants to earn no more than the PM's €176,00 per year, the so-called Balkenende norm.
    But Bekker said not enough consideration is given to how hard the work is, reports news agency Novum. 'Nor is enough consideration given to the difficulties of attracting people to the civil service from the jobs market,' he added.
    Bekker said there is a difference between the political sphere where the salary of the PM, ministers and MPs are set fast, and the professional domain.
    Civil servants should be judged on performance, he told his audience. 'But people prefer not to talk about the productivity of a civil servant, which is exactly why jokes are made about it,' he was reported as saying.

    Are S’Martin’s government officials & MP’s civil servants?
    If yes, is their annual salary not a bit too high for a small country like Sint Maarten?

    Well If Clarence richardson has made over 250.000 GUILDERS IN A YEAR, please do not disclose how much Scot is making Remember he has many more salaries than Clarence
    So it will not be a surprise if he is clocking over 500.000 per year

    So here you have a man sitting in a office only two weeks out of a month, making much more than specialists, who are available for any emergency 24/7, and who have patients lives in their hands

    I am standing a 100% behind our minister of health. Good job minister and see to it that the people of St.Maarten receive quality and affordabel care form this institution.

    I am glad that the SMMC is placed under higher supervision and that they have to start cleaning up thier mess.

    If government if pumping money in the SMMC then they have the RIGHT to question SMMC when necessary.

    The salary of the minister president is 144k per year including vacation allowance and year end bonus, but excluding other perks and allowances. The 176k mentioned (balkenende norm) is 130% of the minister presidents salary. A member of the second chamber is earning 88k excluding 8% vacation allowance and year end bonus. All amounts in euro of course.
